Catalog Request Form

Please note that there is a $5.00 fee for each catalog shipped.  As of September 1, 2003 catalogs will not be shipped until the $5.00 fee has been received. We accept Visa, MasterCard, personnel cheque, and money orders as payment.  If you wish to expedite  your order please fax  your Visa or MasterCard information to (905) 725-2022  please include your name, Visa/MasterCard  number, expiration date, the name as it appears on the card, and we will be glad to ship your catalog today.  

The $5.00 catalog fee will be deducted from your first order over $50.00.
